Every client who attends in-person services will be required to show proof of full vaccination. We strongly recommend clients also have a booster. If you need help scheduling a vaccine or booster, call 314-664-1008 or email info@5starcc.org.

Individuals with a medical exemption from vaccination will still be eligible for at-home services. 

Masks must be worn by all clients, employees, and volunteers while in attendance at the center and in any Five Star Center vehicle. We will have many masks available for client use. 

Individuals with a medical exemption for wearing a mask must provide proof of such exemption from a primary care physician.

Clients must follow all direction arrows, spacing marks, and seating arrangements to ensure everyone is safely distanced from one another. 

Clients must sanitize their hands upon entering the center, after using the restroom, before eating, and any other points throughout the day where contact with another person or community object is necessary.

If a client uses a table top or other common area surface, they may be asked to sanitize it before and after. 

Clients will sign a one-time waiver (view document here) upon their first visit back to the center. Additionally, clients will accept the terms of a daily waiver and give consent for a staff member to check their temperature (daily waiver can be seen here). 

The center staff will clean commonly used surfaces throughout the day. Additionally, the staff will sanitize the entire center at the end of every day with a special machine.
